2017年1月13～15日の3日間、東京都中野区のコングレ中野において、「仮説に則った臨床研究を自ら立案・主導し、日本から世界に向けてエビデンスを発信できる、がん領域における次世代のClinical Investigatorの育成」を目指した “Clinical Research Skill Advancement Workshop” の5回目となるワークショップが開催されました。
　第5回のワークショップでは、欧米の医師主導臨床研究において一般的な「未承認薬・適応外薬を用いた早期臨床試験（第I相もしくは第II相）」の実施を目指し、本ワークショップに共催いただいた製薬企業の開発パイプラインも対象薬の候補とした上で、プロトコールコンセプトシートを参加者から募りました。
　ワークショップには、米国テキサス大学MDアンダーソンがんセンターから、臨床試験の経験豊富な医薬統計学者１名と医師(臨床医、バイオロジスト)2名、日本からはがん臨床試験のリーダー５名と医薬統計学者１名が講師として招聘され、Clinical Investigatorにとって必要な知識についての講義ならびにメンターとしての役割を担っていただきました。
　本会では、3日間にわたってインタラクティブ・セッションが4回行われ、その成果として、参加者は応募時に作成・提出したプロトコールコンセプトシートを完成度の高いものに仕上げるとともに、自身のコンセプトのプレゼンテーションススライドを作成するなかで、クリニカルリサーチデザインの完成プロセスのあり方を学びました。
　以下に、3日間のワークショップの模様を写真とコメントでご紹介いたします。

　“The 5th Clinical Research Skill Advancement Workshop” was held at the Congress-Square Nakano in Nakano-ku, Tokyo, Japan, from January 13th through January 15th, 2017. The objective of the workshop was to cultivate “the next generation of clinical investigators in the cancer research field in Japan” who can formulate and lead hypothesis-driven clinical research and disseminate evidence from Japan to the world.
　At this 5th workshop, we aimed at “early clinical trials (phase I or phase II) using unapproved agents and/or off-label drugs,” which are common in physician-initiated clinical studies in the EU and the US. We solicited the participants for protocol concept sheets using agents including the ones in the development pipelines of pharmaceutical companies that co-hosted this workshop.
　The workshop invited one clinical statistician, one medical oncologist and one biologist with broad experience in clinical trials from The University of Texas MD Anderson Cancer Center (U.S.A.) and five Japanese clinical trial leaders and one clinical statistician as its facilitators. These faculty members offered lectures for clinical investigators and took the role of mentors at the workshop.
　Throughout the three-day workshop, there were four interactive sessions where attendees revised the protocol concept sheets that they had submitted when applying to the workshop. They refined and created presentation slides for their protocol concept sheets. Through this experience, they learned the development process of clinical research design.
The following are highlights of the three-day workshop, including some photographs.

　3日間のワークショップ中、講義とディスカッションからなる4つのワーク・セッションが行なわれました。1日目のワーク・セッションは、「Work 1: Statistical Considerations for Early Phase Trials」、「Work 2: Statistical Considerations for Contemporary Clinical Trials”」の2テーマについて行なわれました。またインタラクティブ・セッションでは、臨床試験の対象薬別に4つのグループに分かれ、参加者は各自のプロトコールコンセプトシートを発表し、グループの皆でディスカッションしました。

　Each day of the workshop included four “Work” sessions of lectures and discussions. The first day’s Work session focused on two topics: “Statistical Considerations for Early Phase Trials” and “Work 2: Statistical Considerations for Contemporary Clinical Trials”. During the interactive session, attendees were divided into four (4) groups by agents, and then presented and discussed their protocol concept sheets in their groups.

　2日目のワーク・セッションは、Work 3 で5つのテーマを取り上げました。 (1) “Bench Side to Bed Side”　(Chandra Bartholomeusz氏)、 (2) “Approach to Rare Cancer & Rare Disease Fraction” （上野直人氏）、 (3) “ICH Guidelines Regarding Clinical Pharmacology for NDA”（今村知世氏）(4) “Immune Checkpoint & Biomarker””（スペシャルゲスト北野滋久氏 国立がん研究センター先端医療科）、(5) “Infrastructure for Investigator Initiated Trial” （鶴谷純司氏）について講義がありました。さらに、「“Post LOI Story”」では、朴成和氏からLOIを製薬企業、関連団体ほかに提出した経験や求められる書類の内容など、実践的な内容についてのプレゼンテーションとディスカッションが行われました。また、インタラクティブ・セッションでは、1日目と同様に対象薬別に4つのグループに分かれ、各自のプロトコールコンセプトシートの革新性・影響力・仮説・リサーチデザイン・実行可能性・プレゼンテーションスキルについて、グループの皆でディスカッションしました。夜のインタラクティブ・セッションでは、各参加者はプロトコールコンセプトシートを完成度の高いものに仕上げるとともに、自身のコンセプトのプレゼンテーションスライドを作成しました。また、“Meet the professors”のセッションでは、参加者の個人面談の機会がもたれ、それぞれの課題について話し合われました。

　The second day’s work session 3 covered five topics, (1) “Bench Side to Bed Side” from Chandra Bartholomeusz, M.D., Ph.D. (2) “Approach to Rare Cancer & Rare Disease Fraction” from Naoto T. Ueno, M.D.,Ph.D. (3) “ICH Guidelines Regarding Clinical Pharmacology for NDA” from Chiiyo Imamura, Ph.D., (4) “Immune Checkpoint & Biomarker” from special guest Shigehisa Kitano, M.D., Ph.D., Department of Experimental Therapeutics, National Cancer Center Hospital and (5) “Infrastructure for Investigator Initiated Trial” from Junji Tsurutani, M.D., Ph.D. .
At the panel discussion “Post LOI Story”, Narikazu Boku, M.D., Ph.D. talked and facilitated the discussion about the experiences submitting their LOI to the pharmaceutical companies and the required documents for submission.
In the interactive sessions, as on the previous day, the attendees were divided into four groups based on agents and discussed innovation, impact, hypothesis, research design, feasibility and presentation skills of their protocol concept sheets in their groups. All attendees refined a protocol concept sheet of their own and created presentation slides of their protocol concept sheet during the evening interactive session. At a “Meet the professors” session, attendees had opportunities for one-on-one meetings and talked about each issue.

　3日目は、参加者全員によるプロトコールコンセプトシートのプレゼンテーションスライドの発表があり、発表ごとに活発なディスカッションが行われました。発表修了後、参加者の質問に答える形でパネル・ディスカッションが行われました。そして最後に修了式が行われ、ファカルティメンバーから各参加者にワークショップ修了証が授与されました。

　On the third day, all attendees presented their presentation slides of the protocol concept sheets, and each presentation was followed by a lively discussion among the participants. After the presentations, all faculty members offered a Q & A panel discussion. The day ended with a completion ceremony at which faculty members were awarded a certificate of completion of the workshop to each attendee.

　Two winners who received high scores by faculty were selected. Selected winners will be invited the clinical workshop at MD Anderson Cancer Center. J-HOPE will give travel grants to the winners.

　J-HOPE faculty hopes that all participants including co-host companies had a fruitful and productive time at the workshop, learning the skills for the development of their careers.
